Key Responsibilities:

• Collaborate to develop client discharge plans

• Assist clients with social and financial care issues

• Communicate client progress to families

• Work within the Neuro Rehabilitation Program

• Engage in interdisciplinary team discussions

Requirements:

• Recognized degree in Social Work

• Registered with Alberta Association of Social Workers

• Minimum of 2 years clinical and continuing care experience

• Preferred experience in gerontology

• Valid Driver’s License
