Geriatric Social Worker

Position: Geriatric Social Worker at AgeCare
Join Age Care as a Geriatric Social Worker, focusing on empowering seniors in long-term care. Provide comprehensive assessments and therapeutic support to enhance residents' well-being and quality of life.

In this role, you will report directly to the Director of Care and collaborate with a dynamic interdisciplinary team. You'll conduct in-depth psychosocial assessments, establish supportive relationships, and offer varied interventions for residents and their families. Your work aims not only to assist in adjustment but also to facilitate resources essential for optimal resident care.

Key Responsibilities:

• Perform comprehensive psychosocial assessments upon resident admission

• Build trusting relationships with residents and families

• Offer counseling and adjustment support to residents

• Provide financial and legal guidance as required

• Employ effective therapeutic communication methods

Requirements:

• Current registration as a Social Worker

• Graduate from an accredited School of Social Work

• Experience with elderly populations

• Strong teamwork ability within a multi-disciplinary team

• Must pass a Vulnerable Sector Check
