Elevate your career as a Social Worker Case Manager with Vancouver Coastal Health in Squamish, BC. This role is key to enhancing the lives of clients through dedicated case management and clinical support.
In this position, you will provide essential social work services to clients experiencing difficulties with social functioning and mood. Collaborating within a multidisciplinary team, you will ensure comprehensive assessments and effective case plans are executed.

Key Responsibilities:
• Deliver psychosocial assessments for clients in need
• Provide counseling and support services to individuals and groups
• Maintain thorough records of client interactions and plans
• Collaborate with other health professionals for client care
• Identify community resources for comprehensive client support
Requirements:
• Master’s Degree in Social Work is required
• Minimum three years in case management for complex needs
• Current registration with the British Columbia College of Social Workers
• Valid BC Driver’s License for community travel
• Strong knowledge in case management and injury prevention
