This part-time Family Supports Navigator role involves helping families identify their strengths and service requirements while overcoming challenges to access support. Collaborate closely with community partners and attend local events to ensure efficient and effective family assistance. You will play a vital role in advocating for improved community service delivery through personalized family support.

Key Responsibilities:

• Conduct intakes and screening with a family-focused lens

• Help families develop service plans tailored to their goals

• Connect families to necessary community resources

• Coordinate service handoffs for smoother transitions

• Regularly follow up to ensure service effectiveness

Requirements:

• Diploma or degree in Human Services or a related discipline

• At least two years of experience working with families

• Strong knowledge of child support services

• Proven record in maintaining documentation

• Understanding of community systems and trauma-informed care
