The Case Manager plays a vital role in providing individualized, participant-centered support to youth and young adults, helping them navigate housing stability and transition successfully into independence. Working from recovery orientated and trauma-informed approaches, the Case Manager provides direct outreach, intensive case management, and system navigation to address complex barriers to housing, mental health, and personal well-being. The Case Manager also works collaboratively within a multi-disciplinary team and in a supportive role with other professional and natural support to ensure wraparound care is provided.

This role will require on call support on a rotating basis, as well as flexibility to support in early morning and evening timeslots as related to case management goals. The successful candidate must have a reliable vehicle, Class 5 drivers license and provide a drivers abstract.

• Manage an assigned case load of 8 youth, including scheduling time to meet with each youth weekly and admin time
• Assist youth in developing and implementing Individualized goals
• Complete intakes, quarterly reports, case notes and other admin duties in a thorough and timely manner
• Provide comprehensive community resource information to youth and their natural supports on individual needs
• Teach and model skills that promote greater self-efficacy
• Transport youth in personal vehicle to appointments in the community
• Complete monthly unit checks
• Utilize a Natural Supports approach to enhance clients' support networks
• Provide support and resources in the areas of recovery, mental and physical health, education and employment and basic needs

Support the organization by participating in a 7 day on-call rotation

• A degree or diploma in Human Services
• Knowledge of youth homelessness and applicable community resources
• Experience working with youth in case management
• Excellent organizational and communication skills
• Strong writing, organizational, program planning and interpersonal skills
• Demonstrated interest and knowledge of Indigenous history, language and culture
• Current Police and Vulnerable Sector Search (must be current within the past 6 months)
• Current Child Intervention check (IRC) (must be current within the past 6 months)
• Current standard First Aid Certification (must be current with the past 3 years)
• Valid Driver’s license and adequate automobile insurance (2 million dollars + 3rd party liability)
• 3 years of driving experience with a Class 5 license
• Suicide Intervention Awareness Training
