The Alex is seeking a Case Manager to work in its innovative Home Base program. This role emphasizes a client-centered, culturally competent approach to providing ongoing support for formerly homeless individuals. Responsibilities include assessing needs, facilitating referrals, and aiding in securing stable housing.
Key Responsibilities:
• Provide case management to individuals exiting homelessness
• Connect clients to community services and referrals
• Conduct comprehensive needs and risk assessments
• Work on daily living skills in clients' homes
• Advocate for client rights utilizing Residential Tenancy Act
Requirements:
• Bachelor’s degree in social work or relevant field
• Experience in the homeless-serving system
• Strong foundation in addictions and mental health
• Exceptional communication and organizational skills
• Valid Class 5 driver’s license and personal vehicle required
