The Role:

The Social Worker plays an essential role within the multi-disciplinary team, providing a range of psychosocial services to residents, their families, and staff members. This role involves supporting residents through a holistic approach, focusing on enhancing quality of life and ensuring the well-being of seniors. As part of the team, the Social Worker will uphold the mission, vision, and values of The Brenda Strafford Foundation, fostering a culture of resident safety, freedom from harm, and ethical practice.

Key Responsibilities:

  • Provide psychosocial support, including counseling, education, and intervention, to residents and their families.
  • Complete comprehensive psychosocial assessments and provide tailored interventions to address residents' emotional and social needs.
  • Work with residents and families on issues such as Guardianship, Personal Directives, Advanced Care Directives, and Power of Attorney.
  • Update Resident Care Plans based on psychosocial assessments, collaborating with residents and families to identify goals and interventions.
  • Maintain ongoing communication with the interdisciplinary team to ensure coordinated care and sensitivity to changing family dynamics.
  • Regularly evaluate the psychosocial needs of residents and families, revising care plans and interventions as needed.
  • Document all interventions and assessments in compliance with legal and organizational standards.
  • Participate in health and safety initiatives and maintain awareness of safety responsibilities.

Who We're Looking For:
  • Maintain active registration with the Alberta College of Social Workers (ACSW).
  • Engage in continuing education and professional development to stay current with social work practices and legislation.
  • Lead or facilitate committees such as the Family Support Group and Residents' Discussion Group.
  • Uphold confidentiality and ethical standards as outlined by The Brenda Strafford Foundation’s policies.

Qualifications:

  • Bachelor’s or Master’s degree in Social Work from an accredited institution.
  • Completion of a recognized Capacity Assessment Course.
  • Current active registration (or eligibility for registration) with the Alberta College of Social Workers (ACSW).
  • Strong interpersonal and communication skills, demonstrating warmth, empathy, and professionalism.
  • Critical thinking and problem-solving skills within the scope of social work practice.
  • Basic computer skills.
