Part Time Social Worker at Park Place

Become a key member of Park Place Seniors Living as a Part Time Social Worker. This role involves supporting new Residents and their families with social work services every week from Monday to Wednesday.

The Social Worker is a vital leadership position at Park Place Home, coordinating all aspects of Resident Move-In processes. You will collaborate closely with the interdisciplinary team, ensuring each Resident’s social, emotional, and spiritual needs are met through tailored programming. Your expertise will guide ethical decisions and improve the care experience for both Residents and families.

Key Responsibilities:

• Lead social work services for new Residents and families

• Conduct assessments for Move-In and Move-Out processes

• Develop and implement Resident care plans with the interdisciplinary team

• Facilitate communication with health services and community resources

• Attend and participate in relevant meetings and committees

Requirements:

• Baccalaureate Degree in Social Work or equivalent

• Experience in social work or elderly care

• Strong verbal and written communication skills

• Exceptional organizational skills and accountability

• Team building and conflict resolution abilities
