Carya's Home Routes Program, in partnership with Astra Living, is seeking a dedicated Home Routes Coordinator. This position requires two years of experience in trauma-informed and person-centered care. You will build strong relationships with participants, guide them through housing resources, and contribute to responsive community programming.
Key Responsibilities:
• Build trusting relationships with participants
• Conduct person-centered assessments and identify barriers
• Assist with referral and application processes
• Connect participants to internal and community supports
• Maintain accurate participant documentation and track outcomes
Requirements:
• Diploma/degree in social work or related field
• Minimum two years’ experience in housing navigation
• Strong communication and organization skills
• Knowledge of Calgary’s affordable housing system
• Valid driver's license and reliable vehicle
