Pruitt Health Hospice is seeking a Flex RN Case Manager to support admissions, peaceful path visits, and routine visits in home or care facility settings. You will coordinate care with the Palliative Care NP and Medical Director, scheduling visits to meet goals of care as ordered by the physician.
Candidates should have 3+ years of nursing experience with RN or LPN licensure in the state. Palliative/hospice or home health experience is preferred, CHPN certification is a plus.
