What You Will Do to Change LivesThe Mental Health Rehabilitation Specialist provides direct community support and case‑management services to assigned members using Psychiatric Rehabilitation principles, a Recovery Philosophy, and Evidence‑Based practices. The role includes education and support for members and families, coordination of service needs, and collaboration with agencies such as the Conservator’s Office, Public Defender’s Office, Child and Adult Protective Services, Probation Department, Inpatient Services, Long‑Term Care Services, Residential Settings, and County Mental Health Services.
The Specialist completes assessments, delivers mental‑health rehabilitation services, crisis intervention, integrated whole‑person care, and trauma‑informed care.
Full‑Time | AM | 8:00 AM – 4:30 PM | Monday – Friday
Expected starting wage range is $26.46 – $32.69. Telecare applies geographic differentials to its pay ranges. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)- Master’s degree in Social Work, Psychology, Counseling, Human Services, or Social Services AND two (2) years of direct services experience in Mental Health; OR Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or Social Services AND four (4) years of direct services experience in Mental Health; OR Associate’s Degree AND six (6) years of direct services experience in Mental Health.
- Valid and current driver’s license and personal vehicle insurance with your name listed as a driver.
- Willingness to use personal vehicle to drive clients to appointments and groups.
- Willingness to use company vehicle to drive clients to appointments and groups.
- Willingness to use personal vehicle to attend meetings.

Eligible employees (20+ hours/week) earn PTO each pay period for vacation and personal needs, with pro‑rated accrual for part‑time schedules and annual carryover up to set caps. - Nine paid holidays & shift differentials for hourly staff (6% for PM Shift, 10% for Overnight Shift). Weekend shift differentials for hourly staff (5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend Overnight Shift).

Age Wise Program OverviewAge Wise is a community‑based program serving older adults. It serves a maximum of 600 individuals aged 59+. Age Wise’s Strengths‑Based Case Management is a consumer‑driven, highly collaborative program. Teams work with members to identify needs and build a treatment plan that is collaborative and comprehensive. The goal is to assist people living in the community and ensure members are linked to housing, medical care, psychiatric care, and natural supports.
Age Wise’s Institutional Case Management serves members who are conserved and placed in a long‑term care facility. The program provides members with support in obtaining and maintaining benefits, care coordination, discharge planning, and advocacy. Members are assessed to ensure placement in the least restrictive environment possible.

QualificationsRequired education:
Associate’s, Bachelor’s, or Master’s degree in Social Work, Psychology, Counseling, Human Services, or Social Services.
