JTC Home Health Inc. is actively seeking a dedicated Physical Therapist to provide high-quality care to patients in their homes throughout the Temecula, Murrieta, and Menifee communities.

As a Physical Therapist with JTC Home Health, you will play a crucial role in our patients’ recovery and well-being. Your responsibilities will include assessing, planning, and implementing rehabilitative programs to help individuals improve their mobility, relieve pain, increase strength, and overcome disabling conditions resulting from injury, illness, or surgery. Working in the home setting allows for a personalized approach, where you can tailor treatments to the patient’s unique environment and daily routine, promoting faster and more effective recovery.

You will also educate patients and their caregivers on exercises, safety measures, and the use of assistive devices to help them regain independence.

• Conduct comprehensive physical therapy assessments and develop individualized care plans.
• Administer therapeutic exercises, manual therapy, and other interventions.
• Monitor and document patient progress, modifying treatment plans as needed.
• Collaborate with an interdisciplinary team to ensure coordinated and holistic patient care.
• Educate patients and their families on home exercise programs and safety precautions.
• Provide a supportive and encouraging environment to motivate patients toward their goals.

Full-time, Part-time, Contract, Per diem

$70.00 - $130.00 per hour

20 – 40 per week
