About my Place Health

my Place Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.

Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our my Place PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.

my Place Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.

About

this Role

This is a wonderful opportunity for a mission-driven team player to make a real difference in the lives of frail and vulnerable older adults in your community. In this role, you’ll bring warmth, kindness, and compassion to every interaction—whether you’re supporting participants with personal care, accompanying them to appointments, or sharing a smile during a group activity. You’ll work closely with families, caregivers, and our interdisciplinary care team (IDT) to ensure each participant feels safe, seen, and supported, while also staying attentive to any changes in their health or well‑being.

  • Help Keep Everyone Active and Comfortable – Provide gentle physical assistance throughout the day using proper techniques to ensure safety and avoid injury for everyone.
  • Pitch in with Food Service – Lend a hand with meal delivery and help create a welcoming and enjoyable dining experience.
  • Take Simple Health Measures – Monitor vital signs and keep a watchful eye on participant well‑being as directed by the care team.
  • Keep Supplies Stocked and Ready – Ensure personal care supplies like incontinence items and grooming tools are available and organized.
  • Follow Team Guidance with Care – Carry out care tasks and status checks based on directions from our interdisciplinary team.

  • A Heart for Service – You’re passionate about caring for older adults and feel called to support high‑risk seniors and those with complex needs.
  • Hands‑On Experience – You have experience as a Personal Care Attendant (PCA) or Certified Nursing Assistant (CNA), especially working with frail or elderly individuals.
  • Safety First – You’re CPR and First Aid certified—or willing to complete certification within 60 days of starting.
  • A Plus, Not a Must – CNA or Home Health Aide (HHA) certification is preferred but not required—we value your experience and heart just as much.
  • Ready to Go – You’re flexible and willing to provide care in participants’ homes when needed.
