Principal Responsibilities:
- Perform comprehensive social work assessments and interventions for inpatients and Emergency Department patients.
- Develop and implement measurable, realistic plans to improve social service delivery.
- Conduct psychosocial assessments and reassessments for all patient populations, including pediatric, adolescent, geriatric, and general patient populations.
- Recognize and respond to psychosocial needs of patients and their families, offering crisis intervention and short-term counseling when necessary.
- Maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, and patients/patient representatives.
- Ensure effective communication at all levels within the hospital system to coordinate social service/discharge planning activities.
- Document patient care activities according to current standards and policies, optimizing patient safety and reducing medical/health errors.
- Provide appropriate resources and support to facility staff. Act as a social services liaison, especially for Advance Care Planning, complex discharge planning, and biopsychosocial assessments.
- Assess behavioral health patients, arrange evaluations, and coordinate necessary follow-up actions.
- Participate in team rounds and interdisciplinary collaborations, assisting with discharge coordination for substance abuse treatment centers and mental health programs.
- Apply clinical judgment to recommend solutions for psychosocial barriers to treatment and discharge planning, coordinating discharge for complex cases and under-resourced patients.
