What You'll Do Evaluate residents and develop individualized treatment plans

Provide skilled physical therapy services in accordance with physician orders and regulatory guidelines

Collaborate with physicians, nursing staff, therapy team members, residents, and families

Monitor patient progress and modify treatment plans as needed

Complete timely and accurate documentation

Educate residents and caregivers on mobility, safety, and recovery strategies

Support residents in achieving maximum independence and quality of life

What We're Looking For Current California Physical Therapist (PT) license

Graduate of an accredited Physical Therapy program

Strong clinical, communication, and documentation skills

Skilled nursing or post-acute experience preferred, but not requiredA patient-centered mindset and passion for helping others succeed
