Post Acute Physician Skilled Nursing Facility Physician

What You’ll Do:

• Round at Skilled Nursing Facilities – Manage chronic and acute conditions for patients in designated Orange County nursing homes.
• Reduce Hospitalizations – Stabilize patients on-site and prevent unnecessary readmissions.
• Lead Goals-of-Care Discussions – Partner with patients and families to ensure treatment aligns with their values.
• Drive Quality & Compliance – Close care gaps, support value-based initiatives, and maintain excellent documentation.
• Collaborate & Lead – Work closely with NPs, nurses, and care teams to deliver seamless, patient-centered care.

Salary & Benefits:

• $235,000 – $295,000 base salary (negotiable based on experience) plus a quality bonus
• Benefits include medical, dental, vision, HSA, matching 401k, generous PTO, and paid holidays

Qualifications:

• MD/DO, licensed in California
• BC/BE in Internal Medicine, Family Medicine, or Geriatrics
• Experience in post-acute or long-term care preferred
• Skilled communicator with knowledge of SNF regulations and Medicare guidelines
