Location: Fillmore, CA
Hourly Rate: $30–$35
Shifts: M–F 7 am–3 pm (Full time)
Sign‑On Bonus: $1,000 ($500 paid after 90‑day probation, the remaining $500 after an additional 90 days)

Job Summary
The Licensed Vocational Nurse (LVN) assists with the coordination of care for residents to meet their health and safety needs. The LVN supports residents to maximize independence and ensures compliance with California Code of Regulations (CCR), Title 17 and Title 22 and the Lanterman Act.
Job Responsibilities
- Administer medications to individuals and verify the five rights
- Attend residents’ doctor appointments and communicate changes with the attending physician
- Assist with activities of daily living—bathing, grooming, meals, cleaning, etc.
- Assess wounds, perform dressing changes and breathing treatments as needed
- Audit all medications during a new medication cycle and check the five rights for verification
- Perform G‑tube insertions for feeding
- Perform colostomy care
- Perform intramuscular and diabetes insulin injections
- Evaluate level of consciousness, neurological factors and responsiveness after seizures or traumatic episodes
- Redirect during behavioral episodes and support residents’ needs
- Complete and update client notes daily and report unusual behaviors or injuries to the Behavior Consultant and Administrator
- Identify early and late signs of infections
- Perform basic physical assessment—vital signs, head‑to‑toe assessments
- Participate in community outing events—hiking, beach trips, leisure activities
- Perform other related duties as assigned
Requirements
- 1 year experience supporting adults with developmental disabilities
- Licensed Vocational Nurse or Licensed Psychiatric Technician (required)
- First Aid, CPR and CPI (Crisis Prevention Intervention) certification
- At least 21 years of age
- California Driver’s License
- Pass all pre‑employment requirements, including background check
