Licensed Vocational Nurse (LVN)

Assess, plan, and implement the nursing care of residents within the skilled nursing setting with supervision. Responsible for administering medications following the eight medication rights to reduce the potential for medication errors. Ensures continuity of care of the residents between shifts by providing direct care and supervising the care given by CNAs and supportive staff. Participates in resident and family teaching.

Maintains role as resident advocate with a focus on the facility's mission. Assesses the resident's condition and nursing needs, prescribing appropriate nursing action. Ability to adequately assess and reassess pain and utilize appropriate pain management techniques. Educates residents and families regarding pain management. Documents changes in residents' conditions and nursing care on progress notes and care plans. Capable of performing waived testing per policies and procedures.

Interprets results of waived tests and takes appropriate action based on results. Administers medications according to the established rights and standards. Meets current documentation standards and policies. Notifies physicians of changes in residents' conditions and follows through until appropriate action is taken. Notes and conducts physician's orders in a timely manner. Provides thorough report to the oncoming shift by participating in rounds and documenting appropriately.

Follows through on information given during shift report regarding resident concerns. Implements the plan of care based on assessments and goals established by the interdisciplinary care team. Treats residents and their families with respect and dignity. Identifies and addresses the psychosocial, cultural, ethnic, and spiritual needs of residents and their families. Supervises other members of the healthcare team as needed to accomplish the plan of care.

Demonstrates competence in technical and manual skills according to scope of practice for LVNs. Functions as a resident advocate by protecting residents' rights. Demonstrates creativity in problem-solving and approaches situations in innovative ways. Interacts with residents, families, co-workers, managers, and departmental personnel in a kind and knowledgeable manner. Consistently involves residents and families in planning for care, treatment, teaching needs, and discharge plans.

Functions as a leader for CNAs and delegates responsibilities appropriately. Participates in orientation of new personnel and assists in performance evaluations when needed. Follows proper infection control procedures and supervises staff in infection control compliance. Demonstrates calmness in emergencies and stressful situations. Consistently follows the policies and procedures of the facility. Exhibits genuine interest in and appreciation of adults in later life.
