LVN​/LPN – LTC​/SNF

Call for Salary Rate –

Job Summary:

Provides skilled nursing care to residents in a sub-acute setting under the supervision of an RN and Director of Nursing. Responsible for assessing, planning, and delivering direct care while ensuring medication safety and effective communication with residents and families.

Responsibilities:

• Assess residents’ conditions, prescribe appropriate nursing actions, and document changes on care plans.
• Administer medications accurately, following the eight medication rights to minimize errors.
• Perform waived point-of-care testing, interpret results, and take necessary actions accordingly.
• Educate residents and their families regarding pain management, care plans, and health education.
• Supervise and collaborate with CNAs and support staff to ensure quality resident care.
• Follow infection control procedures, maintain compliance with policies, and participate in shift handoffs.

Qualifications:

• Graduate of an accredited LVN program with current California LVN licensure.
• Valid BLS certification.
• Proven ability to communicate effectively in English; additional languages preferred.
• Basic computer skills; experience with Electronic Health Records is advantageous.
• Demonstrates competency in scope of practice, including manual and technical skills.
• Ability to remain calm under pressure and show compassion for elderly residents.
