We are seeking an enthusiastic and experienced Advanced Nurse Practitioner (ANP) Independent Prescriber to join our multidisciplinary primary care team.
This is an exciting opportunity for a highly skilled clinician who is passionate about providing high-quality, patient-centred care and helping to shape and develop primary care services.
About the RoleAs an autonomous practitioner, you will assess, diagnose, treat and manage patients presenting with a wide range of undifferentiated and complex health conditions. Working within your scope of practice, you will play a key role in delivering advanced clinical care, supporting service development and contributing to the ongoing success of the practice.
You will work closely with GPs, nurses, healthcare assistants and wider healthcare professionals to ensure patients receive safe, effective and compassionate care.
Key Responsibilities- Independently assess, diagnose and manage patients presenting with acute and chronic conditions.
- Provide advanced clinical assessment and treatment planning.
- Prescribe safely and effectively as an Independent Nurse Prescriber.
- Request, interpret and act upon diagnostic investigations and test results.
- Deliver health promotion, preventative care and patient education.
- Manage long-term conditions including diabetes, hypertension, COPD, asthma and cardiovascular disease.
- Undertake medication reviews and support medicines optimisation.
- Refer patients directly to specialist services when appropriate.
- Support the management of clinical emergencies.
- Contribute to safeguarding, clinical governance and quality improvement initiatives.
- Develop and enhance specialist clinics including well woman and well man services.
- Act as a clinical leader, mentor and role model for colleagues and students.
- Registered General Nurse/Paramedic (currently registered with the NMC or HCPC).
- MSc in Advanced Practice (or training towards).
- Non-Medical Prescribing.
- Current IRMER training and competency in requesting appropriate radiological investigations.
- Experience in management of long term conditions e.g. asthma, COPD, diabetes, CHD.
- Evidence of working autonomously and as part of a team.
- Experience of use of a medical software package.
- Understanding of evidence based practice.
- Proven ability to evaluate the safety and effectiveness of own clinical practice.
- Advanced history taking and clinical examination skills.
- Strong clinical decision-making and diagnostic abilities.
- Proven ability to work autonomously and within a multidisciplinary team.
- Knowledge of clinical governance, safeguarding and evidence-based practice.
- Excellent communication and interpersonal skills.
- Commitment to continuous professional development and service improvement.
