In this replacement position, you will conduct scheduled home visits to provide palliative care for patients with varying complexities. Collaborating closely with Registered Nurses, you will ensure your patients remain comfortable in the community setting.
Your role will require you to assess individual patient needs and adjust care plans accordingly while maintaining professional records.
Key Responsibilities:
• Conduct community visits to assess patient needs
• Provide ethical and safe nursing care
• Utilize assessment tools to evaluate patients
• Monitor and adjust care plans as necessary
• Build rapport with patients to engage effectively
Requirements:
• CNO registration as a Registered Practical Nurse
• Mandatory palliative care experience
• Post-graduate education in palliative care preferred
• Valid driver's license and vehicle access required
• Willingness to travel within designated areas
