As part of the Community Alzheimer Program, you will deliver personalized nursing care alongside a compassionate team. Your responsibilities will include conducting assessments, leading service plan developments, and ensuring the emotional well-being of clients and their families. Key Responsibilities:
• Deliver safe nursing care in day and overnight programs
• Lead service plan development with an interdisciplinary team
• Administer medication while following best practices
• Provide personal care and support during recreational activities
• Conduct regular safety assessments and maintain client wellbeing
Requirements:
• Active registration with the College of Nurses of Ontario
• Experience in gerontological nursing and dementia support
• Approved medication administration certification
• Current CPR training and TB testing
• Excellent skills in team collaboration and client advocacy
