In-Home Health Assessment RN

Activate Care seeks a Registered Nurse to perform in-home health assessments for Massachusetts members under a managed care plan. The role travels to members’ homes, conducts comprehensive evaluations, and documents findings for care management teams.

Training and ongoing education are provided, with emphasis on cultural and linguistic sensitivity. The RN will navigate regulatory requirements, maintain HIPAA compliance, and coordinate with care managers to ensure timely, accurate assessments.
