Medical Social Work Case Manager – Part-time – Cambridge, MA

Conduct psychosocial assessments (in person or via phone) and develop comprehensive outpatient care plans to coordinate medical, social, emotional, insurance, and vocational services for patients and families. Apply case management principles to promote continuous, high-quality, and cost-effective care, incorporate patient-defined goals, and protect privacy and confidentiality. Collaborate with healthcare professionals, consult on clinical/legal/ethical issues, and participate in quality assurance and performance improvement activities.

Master’s degree and a minimum of 5 years of relevant work experience. Licensed Independent Clinical Social Worker (LICSW) or LCSW with a current Massachusetts license. Proficiency in Microsoft Office Suite with intermediate Excel skills. Hospital or healthcare setting experience, knowledge of discharge planning and care coordination, and strong clinical assessment, organizational, and interpersonal skills preferred.
