Base pay range: $43.00/hr - $53.00/hr

We are seeking highly motivated and qualified LPNs to join our nursing department.

LPN Sign‑on Bonus – $6,000 for full‑time, prorated for part‑time.

New Wage Grid – Includes generous shift differentials for evening, night and weekends.

• Assess the health care needs of residents, make a nursing diagnosis, and transition this through collaboration with the nursing team into an interdisciplinary plan of care utilizing all available resources. Communicate the plan to all care providers to ensure continuity of care for residents.
• Establish and maintain functional communication with staff, other disciplines, residents, families, medical staff and community.
• Assess the effect of medications and treatments and monitor these effects to provide appropriate feedback to the interdisciplinary team and medical managers.
• Use the Electronic Health Record – PCC preferred.
• Lead and direct the nursing staff in the safe, effective delivery of care by providing structure. Provide education to residents regarding their health‑care issues and care‑plan strategies.
• Apply knowledge of infection control, fire and disaster policies, and other facility, state and federal policies and practices to maintain a safe environment for residents, community and staff.
• Protect resident rights and treat residents, staff and community with respect and dignity. Handle resident and facility property and information with honesty, confidentiality and respect.
