Job Summary: The Transitional Housing Coordinator is the on‑site coordinator and intensive case manager for a small transitional recovery housing program for individuals in recovery. The Transitional Housing Coordinator contributes to Bay Cove’s mission by partnering with individuals to plan for, and prevent, recurrence, build recovery capital, develop independent living skills, find meaningful employment, and forge a path to permanent housing.
This is an exempt position.
Essential Functions of Position:
- Empower individuals transitioning from substance use treatment to stable, independent housing through the provision of intensive individualized case management, recovery support, and housing navigation.
- Conduct intake, assess needs, and develop, implement, and monitor individualized service and housing plans for each individual.
- Assist individuals in accessing entitlements/financial assistance, employment, healthcare, and other supportive services.
- Facilitate life skills, recovery, and housing readiness activities.
- Oversee daily operations, ensuring adherence to program policies, safety standards, and regulatory compliance, including coordinating with agency property and IT departments to address issues requiring repair.
- Maintain accurate and timely documentation of services and referrals in accordance with agency and funder requirements.
- Coordinate services with external service providers and other stakeholders.
- Collect and report grant data and identify opportunities for quality improvement to enhance outcomes.
- Provide training, coaching, supervision, performance appraisal, and professional development planning with supervises, including progressive discipline as needed.
- Promote a collaborative, trauma‑informed, and recovery‑oriented working and living environment.
- Provide crisis assessment, prevention, and needed intervention during scheduled hours and while on‑call on nights and weekends by providing on‑call coverage on a rotating basis.
- Complete other job related duties as assigned.
Requirements for the position:
- Bachelor’s degree in social work, psychology, counseling, human services, or a related field preferred.
- Minimum of 3 years’ experience in case management, housing services, or substance use recovery programs required.
- Experience working with individuals in recovery and understanding of harm reduction and trauma‑informed care principles.
- At least one year of supervisory or program coordination experience or experience training others is preferred.
- Demonstrated strong interpersonal, organizational, and communication skills.
- Ability to balance direct service with administrative duties.
- Proficiency in documentation and electronic health records/data collection systems required.
- Knowledge of housing resources, recovery supports, and community systems of care.
- Demonstrated commitment to a sustained effort to seek out, acknowledge, and respect the diverse voices of all stakeholders and to advance inclusivity through every function of the role and the agency as a whole.
