Nurse Practitioner – Palliative Care Hospice (PRN)
Come join our family‑owned company! Absolute Hospice is presently seeking a Nurse Practitioner to join our Hospice Team.
Education- Graduate of an accredited School of Advanced Practice Registered Nursing
- End of Life Nursing Education Program (ELNEC) – all 8 modules completed within 18 months of employment
- Current Advanced Practice Registered Nurse license in the state of Ohio
- MSN or Doctoral Degree required
- Certification in Hospice and Palliative Nursing desired
- Minimum of one year experience as an Advanced Practice Registered Nurse, end‑of‑life care experience preferred
- Demonstrated competency in the performance of professional nursing functions
- Excellent oral and written communication skills
- Ability to work with dying patients and their families
- Sympathetic attitude toward caring for hospice patients and positive communication skills with the hospice team
- Accurate assessment of patient eligibility for hospice service
- Valid driver’s license and auto insurance
- Performs comprehensive nursing assessments
- Assess for signs and symptoms of discomfort, including pain, and take prompt action
- Initiates plan of care as needed
- Coordinates all services provided to patients/families in all settings to ensure quality and appropriateness of care
- Provides direct face‑to‑face visits to referred patients and patients already receiving hospice services; contacts physician for updates on patient eligibility and clinical status
- Demonstrates flexibility in scheduling and readily accepts new admissions and emergency assignments as requested
- Maintains knowledge of state and federal regulations and ACHC standards (if applicable)
- Participates in QAPI program
- Observes safety hazards and takes steps to reduce or eliminate them
- Maintains continuing education through academic studies, seminars, and workshops
- Attends and participates in in‑services, staff meetings, and patient care conferences
- Completes all documentation accurately and legibly
- Complies with hospice policies, procedures, laws, regulations, and standards of practice
- Performs all related duties as assigned
