Overview

Advanced Clinical Pharmacist - Antimicrobial Stewardship and Infectious Diseases secondment opportunity

There is now an exciting opportunity to join the antimicrobial stewardship and Infectious diseases (AMS-) pharmacy service as an Advanced Clinical Pharmacist. We are looking for an enthusiastic individual to join the AMS- team and work with the Principal AMS- to continue to develop the service. You will provide an advanced clinical service to the AMS-, as well as assisting the senior AMS pharmacists to provide leadership within the directorate.

You will lead on trust and pharmacy based education and training matters associated with AMS-, as well as on AMS- evaluations and audit reporting. We are looking for an experienced and highly motivated individual, who ideally holds an independent prescribing qualification or is working towards one.

Responsibilities
  • Liaise with the relevant senior pharmacy, medical and nursing staff, other allied health professionals (AHPs) and managers to promote safe, evidence based and cost-effective use of antimicrobials.
  • Provide an advanced clinical pharmacy service to Microbiology, and the Infectious Diseases team
  • Provide specialist pharmacist advice and service to designated wards / clinical areas (AMS/)
  • Act as the nominated pharmacy lead for the development and delivery of training in the specialist clinical area (AMS) for pharmacy staff and other staff groups as required.
  • Act as the nominated pharmacy lead for AMS clinical audits, service evaluations and quality improvement projects
  • Prescribe medication for individual patients within the agreed scope of practice and according to the CUH pharmacist prescribing policy
  • Participate in and lead where required on novel pharmacy projects in the care of patients in designated clinical area (AMS/)
  • Identify and undertake research to inform practice developments and underpin delivery of an effective, evidence-based pharmacy service.
  • Provide pharmacy leadership to the designated clinical area (AMS/)
  • Act as a pharmacist independent prescriber or work towards achieving this qualification
  • Continuously develop and refine models of care through pharmacist-led ward rounds, proactively generating new approaches and adapting existing practices in response to evolving service requirements.
