Location: Fulbourn
Cambridgeshire and Peterborough NHS Foundation Trust (CPFT) is seeking a pharmacist to lead the medicines optimisation within community mental health and assertive outreach teams. The role offers hybrid working and travel across Trust sites to support delivery of patient-centered care.
The successful candidate will join a multidisciplinary team, focusing on improving medicines management and delivering flexible, person-centred support for adults with severe mental health needs.
