Requirements
  • Bachelor's degree in Social Work or Human Services
  • State-required certifications or license
  • 1-3 years of supervised social work experience in a direct healthcare setting - Preferred
  • Additional certifications preferred:
    Geriatric Case Management, Hospice & Palliative Care, Clinical Social Work, Substance Use, etc.
Responsibilities
  • Assist in planning and executing a comprehensive Social Services program.
  • Implement departmental policies and procedures to comply with federal and state regulations.
  • Contribute to the center's Quality Assurance and Performance Improvement process.
  • Maintain timely and accurate patient/resident records, including psychosocial assessments, care plans, and progress notes.
  • Promote and protect patients/residents' rights and psychosocial well-being.
  • Address and report patient/resident abuse, and engage in the Grievance/Concern investigation process.
  • Conduct comprehensive Psychosocial Assessments and discharge planning.
  • Screen for trauma history and mental illness/intellectual disability.
  • Provide therapeutic interventions to support patient/resident adjustment.
  • Facilitate groups for patients/residents and family members.
  • Assist in patient/resident transfers within the center.
  • Provide interventions and staff support during catastrophic events.
  • Arrange and conduct patient/resident family meetings.
  • Collaborate on discharge planning and communication with patients/residents, families, and staff.
  • Develop communication and collaboration with community-based providers.
  • Educate staff on Social Services' role and patients/residents' psychosocial needs.
  • May supervise Social Service students.
  • Other duties as assigned by Management.
