LVN​/LPN – LTC​/SNF

Job Summary:

Seeking a licensed LVN/LPN with extensive PCC experience to provide high-quality patient care in a long-term care or skilled nursing facility setting. The successful candidate will deliver compassionate, efficient healthcare services while adhering to regulatory standards and organizational policies.

Responsibilities:

• Administer patient medications and treatments accurately and promptly
• Monitor and document patient health statuses and report notable changes to the healthcare team
• Follow infection control protocols, including COVID-19 vaccination policies
• Collaborate with interdisciplinary teams to develop and implement patient care plans
• Ensure compliance with facility policies, safety procedures, and regulatory requirements
• Perform clinical tasks within scope of practice to support optimal patient outcomes

Qualifications:

• LVN/LPN license is required and in good standing
• Proven experience with PCC (Point-of-Care Collection) competencies
• Ability to work effectively in long-term care or skilled nursing environments
• Strong attention to detail and excellent communication skills
• Demonstrate reliability in adhering to vaccination and safety policies
• Must bring a valid state or federal issued  wear a photo badge at all times
