HIM Analyst - Lourdes - 1st Shift

Job Summary

Protects and maintains the security and confidentiality of patient data and medical information. Ensures integrity of all clinical documentation images and Master Patient Index by performing prep, scanning, indexing, and quality audits/control of all medical record documents in a timely and accurate manner. Facilitates electronic health record documentation completion by identifying deficiencies and assisting physicians in electronic completion. Handles all aspects of preparation and distribution of information available in the patients’ legal health record as requested by appropriate individuals and hospital departments.

Preserves the integrity of the Legal Health Record and adheres to all policies and procedures.

• Document capture of paper records (prep, scanning, indexing, quality review) into document management system (OnBase), manage electronic images in OnBase and Epic.
• Assess scanned images for accuracy and quality, monitor manual index queues, maintain scanning equipment performance.
• Daily monitoring and completion of Epic In-basket work queues for MyChart and Release of Information requests, and PAT work queues in OnBase for scheduled surgeries.
• Ensure accuracy of demographic information, including patient merges/duplicates and name changes.
• Perform analysis of documentation and assign physician/provider deficiencies for timely completion and regulatory compliance.
• Assist physicians with education and guidance on deficiency completion.
• Validate accuracy of deficiencies on incomplete records, identify and notify physicians eligible for suspension via EPIC EHR.
• Monitor timely completion of medical records and send suspension letter notifications.
• Validate multiple system interfaces to ensure receipt of accurate clinical information into the electronic legal health record; report and escalated discrepancies.
• Assist with monitoring and tracking unbilled accounts and troubleshoot accounts on the Discharged Not Final Billed report.
• Reconcile paper medical records with hospital census to ensure receipt of all patient records.
• Maintain paper record storage areas for accessibility until destruction of original documents; perform final quality control review prior to destruction.
• Ensure flow of clinical documentation to appropriate work queues for internal and external audits.
• Perform and participate in quality audits and reviews; disclose patient information to appropriate parties and ensure release from HIM is in accordance with regulations.

• 2 years of medical office experience required or equivalent in education (minimum of AAS in HIM).
• Strong knowledge of medical record format.
• Detail-oriented and perform tasks at a high level of accuracy.
• Sound decisions and teamwork skills.
• Strong verbal and written communication; customer service focused.
• Preferred: scanning hardware experience; hospital HIM department experience; electronic legal health record system experience; medical terminology knowledge.
