Responsibilities:

• Provides psychosocial assessments, crisis intervention, and resources referrals to facilitate discharge plans and support patients and families during adjustment to illness.
• Formulates the discharge plan with patients, families, and the care team based upon a needs assessment.
• Coordinates appropriate referrals to home‑care agencies, skilled nursing and rehabilitation centers, and community‑based programs.
• Coordinates the care authorization process with insurers.
• Works with the Utilization Management department to ensure appropriate admission status and coordinate specific details of patient hospitalization.

Experience

Required:

3–5 years of healthcare experience preferred.

Education Requirements:

Requires a master’s degree in social work (MSW) or a licensed RN/BSN preferred.

License/Certification Requirements:

• Current NJ SW License; LCSW preferred.
• Or Current NJ RN License.
• ACM (American Case Management Association) preferred.
• CCMC (Commission for Case Manager Certification) preferred.

Special Requirements:

Weekday per‑diem schedule.

Employment Type:

Part‑time.
