Hospice RN Case Manager
Responsibilities- Plan, coordinate and document care for terminally ill patients from admission through discharge.
- Develop and implement a hospice plan that meets each patient’s specific needs using physicians’ orders and professional skills.
- Act as a member of the interdisciplinary team, providing nursing care using the nursing process (assessment, planning, implementation, evaluation).
- Direct and lead other assigned team members and collaborate with multidisciplinary team members to provide age/developmentally appropriate care in accordance with nursing standards of care and practice.
- Provide patient/family centered care using the nursing process and focusing on the physical, emotional, spiritual, cultural, religious, and environmental needs.
- Coordinate the effective and efficient delivery of patient care services through the application of care management principles and identify clinical priorities to improve patient outcomes.
- Registered Nurse licensure by the state of South Carolina or a compact state.
- Bachelor of Nursing degree preferred;
Associate or Diploma degree acceptable with enrollment in an accredited BSN program within two years and successful completion within four years of hire or reclassification. - Minimum of one year of RN experience; one year of case management and/or utilization review work experience preferred.
- Case Management certification preferred; prior leadership experience preferred.
- Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification required.
- Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification required.
- Licensure as a registered nurse by the state of South Carolina or compact state required.
- Certification in Case Management preferred.
