Our therapy team is part of a multi-disciplinary organization serving Northern Arizona from Cottonwood through Sedona to Flagstaff.

The practice of Physical Therapy includes assessment, treatment, planning, implementation, education, communication, and leadership to address patient and family needs and to foster positive working relationships with professionals and ancillary staff. The therapist may care for patients across age ranges and must be able to address physical and developmental needs. The PT will practice within defined standards of care and direct support staff to achieve patient functional and discharge goals, communicating effectively with colleagues to promote a positive problem-solving environment.

• Care Management:
  Participate in patient care conferences with other related health services, patient, and family to review progress and evaluate goals. Develop an appropriate plan of care with goals and home instructions in conjunction with other team members, patient, and family. Delegate selected treatment to supportive personnel with supervision of care and ongoing monitoring.
• Documentation:
  Plan measures to meet learning needs and document in patient records; recognize and address learning needs of patient and family; present information clearly and appropriately; implement the treatment plan and evaluate patient responses; participate in discharge planning; revise/update plans as needed; complete documentation per department policy.
• Education:
  
  Serve as a resource to peers, mentor, and provide educational opportunities; partner with educational institutions to enhance professional development.
• Patient Care:
  Evaluate patients via chart review and diagnostics relevant to the discipline; perform treatments and care activities promptly following appropriate protocols and guidelines.
• Compliance/Safety:
  Report safety incidents, complete required training, stay current with regulations and policies, ensure licenses/certifications are up to date, complete mandatory training, meet productivity goals, respond to orders promptly, and attend department meetings.

• Education:
  
  Bachelor’s degree required;
  Master’s degree preferred.
• Certification & Licensures:
  Current license in the state of Arizona required; BLS required upon hire.
• Certification Specific to Profession:
  Preferred; maintain clinical competencies as directed.
• Experience:
  
  Clinical documentation experience required; general proficiency with computers and electronic systems used in patient care.
