• Provide 1:1 care for clients following the client’s plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed per the plan of care.
• Accurately document nursing actions and communicate with families, case managers, and physicians.
• Coordinate or order supplies, medications, and feedings to ensure adequate availability.
• Ensure all patient needs are met in accordance with physician instructions.

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record and proof of car insurance.
• CPR professional certification.
• 1+ year of nursing experience preferred.
• Home health care experience preferred.

The successful candidate will initially be employed by Amedisys until January 1, 2026. From that date, employment will transfer to Adoration Home Health / Bright Spring Health Services.
