Part‑time. Department: UHN Reactivation Care Centre. Compensation: CAD 38.42 – 39.02 per hour (non‑negotiable).
Join our Talent Pool for Registered Practical Nurses within the Transitional Care Program at UHN Reactivation Care Centre. As a member of a multi‑disciplinary team, the Registered Practical Nurse practices in accordance with relevant legislation/standards, the UHN Professional Practice Model, and the mission and vision of nursing at UHN to provide compassionate, collaborative, and competent care, while meeting the legal and ethical requirements of the profession.
UHN’s Reactivation Care Centre provides a supportive space focused on patient reactivation, helping patients transition safely from acute care to community settings.
Responsibilities- Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in an interdisciplinary context.
- Use current theoretical and research knowledge and input of team members to plan and implement patient and family care.
- Participate in inter‑professional team care planning forums, inter‑professional rounds, and family conferences.
- Provide mentorship to RPN students and new staff on the unit.
- Participate in professional, program, and hospital‑wide initiatives, including personal professional development.
- Ensure compliance with patient safety responsibilities and relevant hospital plans, policies, and procedures to maintain a safe environment.
- Valid registration with the College of Nurses of Ontario.
- Diploma in Practical Nursing.
- Recent related clinical experience preferred.
- Effective problem recognition, problem solving, and decision‑making skills.
- Demonstrated commitment to continuing professional development.
- Demonstrated commitment to team success.
- Resilience and effective coping strategies to manage change.
- Demonstrated computer literacy in Word processing and Electronic Patient Record.
- Working knowledge of the College of Nurses of Ontario standards of practice.
- Evidence of satisfactory attendance and punctuality.
