Geriatric Outreach Nurse

Enhance senior care in the community as a Geriatric Outreach Nurse at Assisted Living Alberta. Provide patient-centered services through assessments and care coordination across various settings in Camrose and surrounding areas.

In this temporary part-time position, you will be pivotal in delivering comprehensive care to frail seniors. Your responsibilities include conducting home visits, cognitive and functional assessments, and creating tailored care plans in collaboration with healthcare teams. You will also engage in ongoing follow-ups and provide clinic support when necessary, requiring significant travel across the designated service area.

Key Responsibilities:

• Conduct home visits to assess senior patients

• Complete cognitive and functional assessments

• Develop and implement individualized care plans

• Collaborate with interdisciplinary healthcare teams

• Support medication coverage applications for clients

Requirements:

• Accredited nursing education program completion

• Active registration with the College of Registered Nurses of Alberta

• Current Basic Cardiac Life Support certification

• Minimum five years' experience in Continuing Care

• Demonstrated ability to work in interdisciplinary teams
