You will expertly navigate the challenges of working with vulnerable populations, conducting prompt assessments and follow-up visits to ensure clients achieve their care goals.

Your role will involve close collaboration with various healthcare professionals.

Key Responsibilities:

• Complete initial assessments within 24 hours of referral

• Conduct follow-up visits to facilitate client progress

• Align care with standards of HCCSS/OH@H

• Create and implement individualized therapy plans

• Educate clients and families on maintaining a safe home environment

Requirements:

• Relevant experience in home health care preferred

• Current registration with the College of Occupational Therapists of Ontario

• Expertise in functional assessments and safety evaluations

• Ability to manage documentation efficiently

• Valid driver's license and reliable transportation required
