For the Hospital Clinical Pharmacist position that is full-time and permanent, recruitment bonuses include: $3,000 on start date, $3,000 at 6 months of service, $5,000 at 1 year of service and $7,000 at 2 years of service. For job shares, salary, bonuses/allowances are pro-rated in accordance with hours worked.

As this is a one (1) year Term position, current indeterminate Government of Nunavut employees may be offered a competitive transfer assignment for the duration of the term if they are deemed to be the successful candidate, however this is subject to managerial approval.

Reporting to the Territorial Director of Pharmacy, the Hospital Clinical Pharmacist is responsible for providing direct patient care to patients at the Qikiqtani General Hospital (QGJ) and Kivalliq Health Centre (KJC) and is a drug information resource for all health centres and long-term care facilities in Nunavut. The Hospital Clinical Pharmacist is an integral member of the health care team and collaborates with prescribers, nursing, laboratory services, engineering/biomedical departments to enhance patient care and safety.

Works collaboratively with pharmacy technicians and assistants to ensure best practices are followed by the department. This position has clinical impacts on medical and nursing staff resulting in an enhancement of patient care due to reduced drug‑therapy problems and overall medication system improvements.

Key Responsibilities

Clinically review medication orders at QGH and KHC for appropriateness and safety.

Provide patient‑specific individualized drug therapy by identifying, resolving, and preventing drug therapy problems through a patient‑focused approach. Assess patients’ medication therapy needs through chart reviews, interpretation of clinical and laboratory findings, and consultation with the multidisciplinary care team to ensure safe and effective treatment outcomes.

Provide specialized clinical pharmacy services such as medication reconciliation, pharmacokinetic/therapeutic drug evaluation/monitoring and discharge planning.

Act as a liaison between the patient care area and the pharmacy, facilitating and resolving medication related issues.

Document clinical activities, interventions, medication therapy recommendations and any other pertinent information in the client health record in accordance with applicable policies and regulations.

Answer complex drug information requests, often in relation to an individual patient care plan. Conduct a systematic search of drug information sources including primary literature and comprehensive databases.

Audit pharmacy services and programs when required and work with Drug Distribution Supervisor to manage safe practices and documentation of controlled substances.

Investigate and resolve medication incidents and complaints, review and revise policies where applicable in collaboration with the Drug Distribution Supervisor and Territorial Director of Pharmacy.

Participate in special assignments and committees.

Travel to Community Health Centres to conduct medication safety audits and provide feedback to SCHPs and regional directors.

May be required to perform other duties as requested.

Qualifications & Requirements

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from a recognized University.

Three years experience providing pharmacist services in a clinical setting.

Pharmacy Examining Board of Canada Certification.

Licensed by, or eligible for licensing as a pharmacist by the registrar of Nunavut.

Pharmacist Registration in a Province or Territory in addition to Nunavut is required.

Completion of Hospital Residency or equivalent combination of education, training and experience preferred.
