Registered Clinical Pharmacist - Primary Care

Location: Omineca and Yellowhead (Smithers)

Contribute to patient wellness as a Registered Clinical Pharmacist at Northern Health. This role combines your clinical knowledge with patient interaction to resolve drug therapy issues.

Northern Health seeks a dedicated Clinical Pharmacist for its Primary Care team, responsible for delivering clinical pharmacy services within the primary care network. You will work alongside an interprofessional team, directly interacting with patients to assess and solve any drug therapy challenges. This role operates during weekdays, 08:30 to 16:30, with schedule flexibility based on the organization's needs.

Key Responsibilities:

• Provide direct clinical pharmacy services to primary care patients

• Identify and rectify drug therapy problems collaboratively

• Work closely with multidisciplinary healthcare professionals

• Oversee comprehensive medication management practices

• Flexibility in scheduling will be necessary on occasion

Requirements:

• Valid registration as a Clinical Pharmacist

• Strong interpersonal and teamwork abilities

• Experience working in medication management

• Willingness to adapt to operational changes in shifts

• A passion for patient advocacy and healthcare improvement
