Community Inclusion Specialist at PCCR

Location: North Island, Sunshine Coast, and Southern Gulf Islands (Whistler)

Transform lives by becoming a Community Inclusion Specialist with Pacific Coast Community Resources. Support clients in achieving independence and inclusion in their community through tailored assistance.

In this vital role, you will help your clients navigate community engagement through personal, social, and vocational activities. You will also advocate for their choices and rights, ensuring a supportive environment that promotes their well-being. Your interpersonal skills will be essential in fostering collaboration with families and other community members.

Key Responsibilities:

• Advocate for the rights and choices of the supported individual

• Facilitate their involvement in community activities

• Build effective partnerships with families and professionals

• Document daily activities and health information accurately

• Engage in the development of personalized service plans

Requirements:

• Two years of experience in community social services

• Grade 12 education plus Community Support Worker Diploma

• Current First Aid/CPR and Food Safety certification

• Ability to communicate effectively in various settings

• Valid BC Driver's License and reliable vehicle with insurance
