Scope of Practice (Services)
As a Clinical Pharmacist you will work to provide the following services to our patients:
Individualized pharmacotherapy
Counselling patients about their medication therapy
Work in collaboration with physicians and other health‑care providers to develop, implement and monitor therapeutic plans
Assist in the development of drug therapy guidelines for individual disease conditions
Prepare and submit case studies and articles for publication in professional journals that share new knowledge
Act as department resource person within your assigned area
Participate in medication‑use evaluations, quality‑improvement initiatives and clinical drug trials
Provide mentorship and supervision to pharmacy students, residents and new staff
Ensure prescriptions are validated and dispensed accurately; work with the health‑care team to resolve medication‑safety concerns and discrepancies promptly and document professional‑practice activities proficiently
Responsibilities
Provide consultation services to internal and external health‑care professionals and apply advanced theory to assess, plan, implement and evaluate pharmaceutical care in the designated specialty area.
Provide clinical direction on complex cases, including evidence‑based evaluation of drug therapy, attend patient‑care rounds, review medication histories, provide drug information, patient education and discharge consultations, and recommend treatment policies and procedures.
Ensure quality‑assurance programs are maintained, identify and troubleshoot inconsistencies, develop standards, conduct regular audits and take corrective action. Participate in quality‑improvement activities and recommend program modifications to the Director of Pharmacy Services.
Provide academic teaching to undergraduate and post‑graduate students, plan and evaluate hospital‑based Pharmacy programs in cooperation with the Faculty of Pharmaceutical Sciences, University of British Columbia.
Initiate and oversee clinical drug research, develop hypotheses, coordinate research assistants, analyse data, evaluate findings and write articles for submission to scientific journals.
Conduct pharmacological education and in‑service sessions in the designated specialty area and orientation sessions for pharmacy staff and other health‑care staff.
Participate in the Pharmacy and Therapeutics Committee and provide necessary information for formulary decisions and policy‑making.
Participate in planning, implementation and evaluation of department therapeutic cost‑effectiveness and cost‑restraint programmes; make recommendations to the Director of Pharmacy Services.
Develop, implement and evaluate goals, objectives and outcomes for the program team and related policies for the designated specialty area.
Represent the department on multidisciplinary care committees and at external events such as conferences and regional planning groups.
Provide pharmaceutical care to patients in the designated specialty area, review patient charts, identify and solve drug‑related problems, monitor therapy and outcomes, maintain medication profiles and educate patients and families.
Perform other related duties as assigned.
Qualifications
Education and Experience
Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
Certificate of completion of year 1 and year 2 of a Hospital Pharmacy Residency Programme accredited by the Canadian Pharmacy Residency Board (CPRB) supplemented by two (2) years’…
