As a Clinical Pharmacist you will work to provide the following services to our patients:
Individualized pharmacotherapy
Counselling patients about their medication therapy
Work in collaboration with physicians, and other health care providers to develop, implement and monitor therapeutic plans
Assist in the development of drug therapy guidelines for individual disease conditions
Prepare and submit for publication in professional journals, case studies and articles that share new knowledge
Acts as department resource person within your assigned area
Participate in medication use evaluations, quality improvement initiatives and clinical drug trials
Provides mentorship and supervision to pharmacy students, residents and new staff.
Additionally, we would depend on your expertise to ensure that prescriptions are validated and dispensed accurately; and that medication safety concerns, working with the health care team to resolve discrepancies and/or problems are resolved expediently; your professional practice activities shall be documented proficiently.

Detailed Overview
Situated within identified Correctional Health Services locales, this position provides clinical pharmacy services to clients of the Correctional Health Services. As a member of an interprofessional team, this position works directly with clients to identify and resolve actual and potential drug therapy problems through provision of comprehensive medication management.
Responsibilities
Identifies and resolves actual and potential drug therapy problems in collaboration with the client and other members of the health care team.
Provides comprehensive medication management and pharmaceutical care by monitoring the medical condition(s) and ongoing drug therapy for effectiveness, adherence, adverse reactions, drug interactions and therapeutic, chemical and laboratory incompatibilities. Communicates pertinent findings, recommendations and plans to the client and appropriate healthcare practitioners.
Consults with clients and members of the health care team to develop client‑specific therapeutic plans based on chart review, client information and assessment of clinical responses.
Maintains appropriate documentation in the health record in accordance with organizational standards and practices as required.
Uses appropriate resources to support professional practice and client care including those available through provincial programs, such as academic detailing, optimal drug use initiatives, opioid stewardship and de‑prescribing.
Establish linkages, as required, with health authority, community pharmacy services and other relevant community services to support continuity of care for clients.
Provides drug information, consultation and other support services to medical, nursing, pharmacy and other health care team members by communicating with them on drug related issues, questions and client specific therapeutic regimens.
