BHFN is hiring a Community Health Representative to support health program delivery and community wellness.
Brunswick House First Nation is accepting applications for a Community Health Representative
.
The Community Health Representative will help keep community leaders and members informed about current program delivery and encourage community input and ideas to enhance the delivery of the health program.
This program will be delivered by a community-based CHR in conjunction with the NNADAP Worker, Community Health Nurse, and Mental Health Worker.

- Position: Community Health Representative
- Starting Salary: $25/hour
- Reports To: Health Manager
- Assist and participate with the Community Health Nurse in offering prenatal, maternal, well-baby, childcare, preventive health clinics, and home visits.
- Meet regularly with health care providers and/or health committees to enhance the delivery of ongoing health programs.
- Prepare materials for relevant workshops to promote healthy lifestyles for community members.
- Coordinate medical transportation in conjunction with the First Nations & Inuit Health / NIHB program to support the continued health and well-being of community members.
- Assist the Community Health Nurse with ongoing updates of client core data for the Health Information System to reflect current community statistics.
- Maintain accurate statistics based on client encounters, client referrals, home visits, emergency after-hours treatment, program activities, and related work.
- Grade 12 or equivalent with health education background relevant to the position.
- Demonstrated healthy lifestyle and ability to be a positive role model for the community.
- Experience working with Electronic Medical Records programs is considered an asset.
- Strong interpersonal skills, including the ability to establish and maintain effective working relationships with clients, families, management, and staff.
- Ability to work as a team member and independently as a self-directed worker.
- Experience with Microsoft Word and Outlook.
- Ability to write or complete required reports.
- Good verbal and written communication skills.
- Valid driver’s licence and access to a reliable vehicle for work-related responsibilities.
