Position: Case Coordinator for Home Care Services
Location: South Central Manitoba (Altona)

Join Southern Health-Santé Sud as a Case Coordinator, providing essential home care services in Winkler. Deliver comprehensive client assessments and resource coordination through a client-focused approach.

The Case Coordinator takes charge of assessing and implementing care plans while working alongside a multidisciplinary team under the guidance of the Manager of Health Services. Ensure services address client needs and support effective resource utilization while compliance with health standards.

Key Responsibilities:

• Collaborate on client assessments to determine health service needs

• Develop and implement coordinated care plans efficiently

• Monitor client progress and adapt services as necessary

• Communicate effectively with clients and other service providers

• Maintain adherence to health policies and regulations

Requirements:

• Minimum two years of related experience in case management

• Graduate from a relevant post-secondary program required

• Must hold an active license to practice

• Strong organizational and communication skills

• Bilingual candidates preferred for certain roles
