Care as a Psycho-Social Support Coordinator, ensuring smooth resident transitions and providing ongoing support to residents and families. Key duties include managing admissions and facilitating care conferences.
As the Resident and Family Coordinator, you will coordinate admissions and engage directly with new residents to provide psycho-social support.
Your role includes collaborating with care teams to enhance resident well-being and managing family-related concerns efficiently.
Key Responsibilities:
• Manage admissions, applications, and occupancy trends
• Facilitate tours and orientation for new residents
• Conduct initial and annual care conferences
• Provide emotional support during discharge processes
• Collect feedback through satisfaction surveys
Requirements:
• Registered Social Service Worker (RSSW) designation mandatory
• Relevant healthcare experience preferred
• Effective communication and organization skills
• Experience in leadership or coordination roles
• Familiarity with long-term care legislation
