Position: Health Navigator for Taykwa Tagamou Nation
Location: Northeastern Manitoulin and the Islands

Shape health outcomes within Taykwa Tagamou Nation as a Health Navigator/NHIB Service Worker. Utilize your interpersonal skills and health knowledge to assist community members in navigating healthcare services.

Taykwa Tagamou Nation seeks a motivated professional to join its Health Department. In this role, you will coordinate outreach strategies and work closely with community members. Key responsibilities include providing health navigation services and collaborating with nursing teams to improve healthcare access for all ages.

Key Responsibilities:

• Assist clients with medical appointment navigation and care planning

• Engage clients with empathetic, non-judgmental communication

• Assess client needs and develop culturally safe solutions

• Collaborate with nursing health teams and e-health services

• Build relationships across community services and organizations

Requirements:

• Ontario Secondary School Diploma or equivalent

• Minimum two years’ experience in health or community work

• Knowledge of Aboriginal culture and experience working with Aboriginal peoples

• Valid G driver’s license

• Strong organizational and time management skills
