Position: NHIB Service Worker Role in Health Navigation
Location: Northeastern Manitoulin and the Islands

Empower community health as an NHIB Service Worker with Taykwa Tagamou Nation. This position focuses on coordinated outreach and enhancing health access for all community members.

Join the Health Department of Taykwa Tagamou Nation as a Health Navigator. You'll take a proactive approach in connecting individuals with necessary health services.

Your role will encompass assessing client needs, providing digital health access, and nurturing relationships with various health and community programs.

Key Responsibilities:

• Navigate clients through medical appointment processes

• Utilize trauma-informed practices to assess client needs

• Collaborate with nursing teams for care solutions

• Engage clients with supportive and culturally sensitive communication

• Identify models of service delivery for improved access

Requirements:

• Minimum of Ontario Secondary School Diploma

• At least two years’ experience in relevant sectors

• Familiarity with NHIB policies and procedures

• Must hold a valid G driver’s license

• Advanced Microsoft Office skills required
