Southern Health Community Health Facilitator

Location: South Central Manitoba (Altona)

Make a difference in community health as a Healthy Living Facilitator with Southern Health, focusing on health promotion and community engagement strategies. This role requires program development and collaborative communication.

As a facilitator in the Public Health-Healthy Living team, you will work to enhance community well-being through health initiatives that prioritize health equity and population health promotion. Key tasks include implementing, evaluating, and developing health programs while supporting effective communication with community stakeholders.

Key Responsibilities:

• Identify community health initiatives and evaluate their impact

• Develop health promotion strategies in collaboration with the team

• Maintain communication with staff and community stakeholders

• Work independently on community-related tasks

• Engage with diverse populations to promote inclusivity

Requirements:

• At least two years of experience in population health promotion

• Training in mental health promotion preferred

• Relevant post-secondary degree in health discipline

• Proficiency in Microsoft Office and Outlook applications

• Valid Class 5 driver’s license and liability insurance
